When to use this Art Class Registration Form

  • Materials supplied or bring-your-own Single select + payment

    "Materials included in fee" or "I'm bringing my own kit." The included option triggers a materials-fee line on the payment field. The BYO option triggers a supply list email through the auto-responder.

  • Allergies, sensitivities, and accessibility needs Long text

What is an art class registration form?

The studio or instructor uses it to fill sessions, build the roster, charge the materials fee, and have a signed release on file before day one. It replaces the paper signup at the front desk, the Venmo request, and the chasing-down-permission-slips email.
Does the template collect the materials fee and tuition payment?
Yes. Connect the form to Stripe or PayPal and add a payment field. Use a checkbox for the tuition tier (single workshop, six-week course, monthly drop-in) and a separate line for the materials fee, so the total updates live as the student picks options. The seat only locks when payment clears, so the studio never has to chase money before the first class. Refunds are processed from the same dashboard.
Can parents e-sign the photo release for minors?
Yes. When the student picks an age band under 18, conditional logic shows a parent or guardian contact block and an e-signature field for the photo release, the studio's safety policy, and any waiver you carry. The signed PDF saves to the submission record so it is one click away on the day of class. For adult students, the same e-signature block appears under their own name without the parent step.
Do I need to bring my own materials, or are they supplied?
That depends on how you set the form up. The template ships with a single-select field ("materials included in the fee" or "I'm bringing my own kit"). The "included" option triggers a materials-fee line on the payment field; the BYO option triggers a supply list emailed through the auto-responder so the student knows exactly what to bring. Most studios run kids' classes materials-supplied and adult intermediate classes BYO; the form supports both side by side.
Can I cap enrollments per session?
Yes. Set a per-session submission limit on the form. Once the cap is hit (say 10 seats for Wed 6pm pottery), that session option hides itself and new students see a "this session is full" message, while other sessions stay open. You can also schedule the entire form to close on a specific date so late enrollments cannot squeeze past your start-of-term deadline.
Will students get a confirmation email after they enroll?
Yes. Turn on the auto-responder and every enrollment receives a confirmation with the class details, the session they picked, the supply list (if BYO), and a calendar attachment. Parents of minors get a separate copy CCed in if you switch on the parent-contact branch.
How do I export the roster for the first day of class?
Filter the dashboard by session, age band, or skill level, then export to CSV. Hand the roster to the front desk on day one with allergies and accessibility notes flagged. For multi-session studios, filter by the specific session date so each instructor only sees their own class, not the full week's roster.
How do I clone this form for the next term or workshop series?
