Appointment scheduling form builder

Build an appointment scheduling form in minutes

One form does the whole job. The client fills out intake details, picks a slot, uploads anything you need, and pays if you charge. The booking lands in your calendar, the intake lands in your CRM.

What an appointment form does that a Calendly link cannot

Pick a slot is the easy part. The work happens around the slot.

Collect the intake before the booking is confirmed

New-patient history, project brief, support context, dietary needs. Anything you would normally email about after the slot is booked, ask for in the form. The slot only confirms when the form is complete.

Qualify before you spend a calendar slot

Add a conditional question that hides the slot picker until the prospect meets a baseline (location, budget, role, urgency). The unqualified ones never reach your calendar.

Charge for the booking when it matters

Stripe and PayPal built in. Take a deposit for a session, a no-show fee for consultations, a full payment for paid coaching. The slot books only when the charge clears.

Capture files at booking

A photo of the issue, an X-ray, a contract draft, a reference. Upload alongside the booking; the file shows up in your dashboard before you walk into the room.

Send a custom confirmation, not the generic Calendly email

The autoresponder uses the form data: "Hi Maya, your dermatology consult on June 4 at 2 PM is confirmed. Bring the photo you uploaded and your insurance card." Real personalization, no template fishing.

Sync the slot to Calendly automatically

If you already run Calendly for your team calendar, the appointment-scheduling form respects your existing availability, syncs the booked slot back, and keeps double-bookings impossible.

Three steps to a working appointment scheduling form

No code. No second tool. Five minutes from sign-up to first booking.

Drop the Scheduler field into your form

Open the form builder, find the Scheduler field, drag it in. Add the intake questions you need above the scheduler: name, contact, reason for the visit, file uploads, anything else.

Connect your calendar

Paste your Calendly link into the Scheduler field. Formester syncs your availability and surfaces only the slots you have open. If you do not use Calendly today, you can still publish the form; the scheduler shows fixed time slots you define directly.

Publish and share

Publish the form to its own URL or embed it on your site. Share the link. Every booking lands in your Formester dashboard with the full intake, the booked slot, and any files attached. Your existing calendar picks up the booked slot at the same time.

When a form-based appointment beats a bare booking link

Six scenarios where the difference shows up in the first week.

Healthcare

New-patient intake at a clinic

Without a form: patient books the slot, receptionist emails them an intake form, half of patients show up without filling it. With the form: intake is the booking step. The clinic walks into every appointment fully briefed.

Sales

Service-business consultations

The consultant needs to know the project type, budget, and timeline before saying yes to a 30-minute call. A bare booking link gives them nothing. A form filters out the unqualified leads and books the qualified ones.

Multi-step

Routing to the right slot type

"Phone screen first, then in-person if it goes well." A form routes the booker to the right slot type based on their first answer. No second booking link.

Paid

Paid bookings

Coaching sessions, premium consultations, deposit-required services. A bare booking link routes to a separate payment tool. A form does both in one step.

Global

Time-zone-sensitive bookings

The form asks the booker's location first, then shows the scheduler in their time zone. No "what time zone is that?" confusion in the confirmation email.

Files

Bookings that need a file

Tax consult (last year's return), legal intake (the contract), creative review (the brief). A form captures the file at booking; the booker is never asked to "reply with attachments" afterward.

Appointment scheduling form vs. bare booking app

Pick whichever fits the job.

Takeaway: If the booking is "just pick a slot, that is enough," a bare Calendly link is fine. If the booking comes with intake data, qualification, files, or payment, a form-based appointment beats both Calendly alone and the Google Forms + Calendly hack.

Formester offers a ton of features and my developer loves using it to integrate to various marketing pages. It's quick and easy to create using drag and drop. It allowed me to remove branding unlike Typeform that has branding even in paid plans. Their support is top notch.

Common questions

Should I use this or just send a Calendly link?

Use a Calendly link if all you need is the slot. Use an appointment form if you also need intake data, qualifying questions, file uploads, payment, or a custom confirmation email. The form layers everything around the slot in one step.

Do I need a Calendly account?

No. The Scheduler field can show fixed time slots you define directly in Formester. Connect Calendly if you already use it for team calendar management and want availability synced automatically.

Can the booking only confirm after payment clears?

Yes. Add Stripe or PayPal to the form. The slot books and the confirmation email fires only when the charge succeeds. Useful for deposit-required services, paid consultations, premium coaching.

What does the booker see in their confirmation email?

By default, a clean confirmation with the slot details. Edit the autoresponder to use any form field: "Hi {{first-name}}, your {{service-type}} on {{slot-date}} is confirmed. {{custom-instructions}}." Variables pull from the actual submission.

Can the form handle different appointment types?

Yes. Add a "What kind of appointment?" question at the top with conditional logic. Each answer can show a different scheduler with different available slots, different intake questions, and a different price.

What time zones does the scheduler support?

The scheduler shows the booker's local time automatically. Detect the time zone from the browser, or ask the booker explicitly at the top of the form for a more controlled experience (recommended for high-stakes bookings).
