A basketball signup sheet is the one place you collect who's playing, what size jersey they wear, what position they play, and which nights they can make it. It replaces the paper sheet at the gym, the group-chat thread, and the half-finished spreadsheet on someone's desktop. League organisers, coaches, and rec-centre staff use it to build rosters, balance teams, and email parents in one shot.
The right form is a sheet, not a contract. Most signups don't need a waiver, a payment, or a bracket. Keep them out by default and the form stays a 60-second fill. Choose your level
The template ships level-agnostic on purpose. Pick the level you run and tune the field set in two minutes.
High school
Add grade, height, position, and a previous-year team field. Coaches use the previous-team field to flag transfers and varsity-eligible players.
College or club
Add student ID, year, position, and a "rec or club" toggle. Add a Discord or WhatsApp handle so team comms land in the right channel.
Adult recreational
Add self-rated skill (1-5 or beginner / intermediate / advanced), preferred night, and a notes field for injuries or work-travel weeks.
Women's league
Same field set as adult rec, with a women's-league dropdown at the top. Captains can also collect a sub-list field for game-night fill-ins.

What is a basketball signup sheet?
A basketball signup sheet is the one online form a coach, league, or rec-centre uses to collect who's playing. It captures player name, contact, jersey size, position, availability, and level in one submission. Organisers use it to build rosters, balance teams, order kits, and email the right player when game time changes.
What fields should a basketball signup sheet include?
At minimum, capture player name, mobile, jersey size, position, availability, and level. For youth signups, add parent / guardian name and a parent mobile. For high school and college, add grade and height. For adult rec, add a skill-level dropdown. Leave waiver and payment off by default, those belong on the Tournament Registration Form for bracket events.
Can I register both individual players and teams in one form?
Yes. Add a single-question toggle at the top of the form (Individual or Team) and use conditional logic to show the right block of fields after the answer. Individuals see a streamlined player block. Team captains see a roster block. The form stays one URL, so you only share one link.
Can I collect entry fees on a signup sheet?
You can, but most signup sheets are fee-free by design. If you need to charge a season fee or per-session drop-in, connect the form to Stripe or PayPal and add a payment field. The player only locks their slot when payment clears. For bracket tournaments with entry fees, switch to the Tournament Registration Form template.
How do I manage basketball signups after players register?

Can I cap the number of signups or close the form once a team is full?
Yes. Use the form submission limiter to set a per-level or per-team cap. Once the cap is hit, the field hides itself and new entries see a "this level is full" message, while other levels stay open. You can also schedule the entire form to close at a registration deadline.
How do I clone this form for next season?

Make jersey size required only when the league orders team kits, and make position optional for casual pickup games. For high school and college signups, add a "primary / secondary position" pair so a forward who can swing to centre gets logged correctly.
