About this template

Career counsellors and coaches start every new client the same way: a one-hour first session where half the time disappears into "what's your background?" instead of "what do we do next?" This intake form template flips that. The client fills it in before session one, the answers land in your dashboard, and you walk into the call already knowing their education, current role, target industries, salary band, and what's blocking them.

It works for full-time counsellors, side-of-desk college guidance counsellors, HR outplacement teams, and executive or life coaches who run paid 1:1 packages. If you also need a pre-session assessment, pair this with the Career Suitability Assessment. If you run mental-health or general counselling rather than career-specific work, switch to the Counseling Intake Form instead.

HR and outplacement teams

Onboard laid-off employees into the outplacement programme. Collect last role, tenure, severance window, and target re-entry timeline. Route each intake to the right coach based on industry.

Executive and life coaches

Pre-fill the discovery call. Collect current title, decision-making scope, the goal the client paid for, the obstacle, and a multi-session preference. Book the call inline.

University career centres

Triage drop-in appointments before the first slot. Collect year, major, target industries, prior internships, and whether the visit is resume / interview / job-search. Auto-route to the right counsellor.

Career-change coaches

Map a mid-career pivot. Collect current sector, target sector, transferable skills, salary band the client will accept, and the timeline they have runway for.

    First name, last name, email, mobile, preferred pronouns, time zone. For under-18s, swap or add a parent / guardian field.

  • Current role and tenure Text + dropdown

    Current job title, employer, years in role, years total in the industry, employment status (employed / between roles / student). One short-text + two dropdowns.

  • Education Multi-select

    Highest qualification, institution, year completed, current credentials in progress. Multi-select for additional degrees.

  • Skills inventory Multi-select + 1-5

    Top five technical or domain skills the client owns. Optional 1-to-5 self-rating per skill. Open-text for soft skills they want to grow.

  • Career goals and timeline Long text + dropdown

    Short paragraph on the goal they're hiring you to help with, a dropdown for timeline (0-3 months, 3-6, 6-12, 12+), and a dropdown for goal type (new role, promotion, pivot, returnship, business).

  • Target industries and roles Multi-select + text

    Multi-select of target industries plus 3-5 target job titles. Helps you pre-shortlist openings or alumni intros.

  • Salary expectation band Dropdown

    Dropdown of bands (Under $40k / $40-60k / $60-90k / $90-130k / $130-200k / $200k+). Optional "negotiable below this band" toggle.

  • Consent and file-upload e-Signature + file

    Confidentiality acknowledgement, e-signature block, and a file-upload field for CV, transcripts, references, and any prior assessment results.

Is client information collected through this form confidential?

How do I follow up after a client submits the intake?
Set an autoresponder email that fires on submission with a short "thanks, here's what to expect in session one" note and the calendar confirmation. Set a separate email notification to yourself so you see new intakes the second they arrive. For multi-session series, set a calendar reminder on the day of session one to re-read the intake before the call. Most counsellors filter the dashboard by submission date and triage Monday morning.
Can I reuse the same form across a multi-session series?
Yes. The intake is a session-one artifact, not a per-session form. Take the answers as the baseline record for the whole series. For mid-series check-ins, clone a shorter "session check-in" form (3-5 fields: progress since last session, what changed, next focus) and link it from your session-reminder email. For end-of-series reflection, clone a "wrap-up" form. Each client keeps one intake record plus a thread of check-ins.
Can I customise the form for executive coaching vs college guidance?
Yes. Use conditional logic to show different blocks based on a "What kind of session is this?" dropdown at the top. Executive coaching clients see decision-making scope, target salary band, and the obstacle; college guidance students see grade, GPA range, target majors, and parent / guardian contact.
