Career counsellors and coaches start every new client the same way: a one-hour first session where half the time disappears into "what's your background?" instead of "what do we do next?" This intake form template flips that. The client fills it in before session one, the answers land in your dashboard, and you walk into the call already knowing their education, current role, target industries, salary band, and what's blocking them.
It works for full-time counsellors, side-of-desk college guidance counsellors, HR outplacement teams, and executive or life coaches who run paid 1:1 packages. If you also need a pre-session assessment, pair this with the Career Suitability Assessment. If you run mental-health or general counselling rather than career-specific work, switch to the Counseling Intake Form instead.
