Set up the workspace connection once. After that, every published form gets its own Delta table and writes responses on its own.

Add Databricks from the Automate tab

Open your form, head to Automate → Integrations, find the Databricks card, and click Add integration.

Add your workspace and warehouse details

In Databricks, open your SQL Warehouse's Connection details and copy the Workspace URL and SQL Warehouse ID. Then pick the Unity Catalog and schema where Formester should create tables, and paste all four into the panel.

Add a service principal, then connect

Create a service principal in Databricks with SQL and workspace access, generate an OAuth secret, and grant it privileges on your catalog. Paste the Client ID and secret into Formester and click Connect. From there, each published form gets its own table and new responses are added as rows.
