An auto repair release form authorizing a shop to perform work on a vehicle. Captures vehicle details, scope of work, parts authorization, and signed release. Exported as a signed PDF.

Releases & Waivers

About this template

This auto repair release form template runs on Formester's drag-and-drop form builder, sized for independent auto shops, dealerships, and mobile mechanics. Capture the vehicle details, scope of work, parts authorization, and signed release before the lift goes up.

Replace the paper work-order pad. Customers fill the form at the counter (or text the link from the parking lot), sign with e-signature, and the signed PDF is in the customer file before the bay door closes. Encrypted in transit and at rest.

Use conditional logic to surface a tear-down-fee block when diagnostic work is needed, reveal the storage-fee terms for vehicles left overnight, or scope a separate towing-release block when the vehicle was towed in.

Auto-send the signed release to the customer via Autoresponder Email, route every release to your shop-management software via webhooks, and skip the data re-entry.

How to customize this auto repair release form

Six steps to a shop-ready release form.

Brand the header

Replace the placeholder organization name, logo, address, and contact details with your brand. Set primary colors so the form matches the page it lives on.

Configure vehicle and scope of work

Add the make/model dropdowns (or free text), VIN field, scope-of-work multi-select (diagnostic, repair, replacement, inspection), and an estimated-completion-time field.

Set the estimate-and-overage policy

Edit the estimate-overage threshold (e.g., +15%). Above this, the customer is re-prompted to authorize the additional cost via the same form (or a separate one).

Add storage and tear-down fee terms

Surface the storage-fee block for vehicles left more than 24 hours, and the tear-down-fee block for diagnostic work that doesn't proceed to repair.

Wire delivery

Push every submission to your CRM or e-vault through a webhook, or use Zapier for fan-out to email, Slack, and notifications. Turn on Autoresponder Email if the signer should receive a copy.

Publish and share

Embed the form on your site, share the direct link by email, or generate a QR code for in-person signing. Every submission lands in the dashboard with a clean PDF for the file.

When to use this auto repair release form

Six shop scenarios this template handles.

Routine repair appointment

Customer signs the release at drop-off. Captures the work scope and the estimate.

Diagnostic work

Tear-down-fee block surfaces. Customer authorizes the diagnostic fee even if they don't proceed to repair.

Towed-in vehicle

Customer pre-authorized towing; this form captures the repair-side authorization once the vehicle arrives at the shop.

Remote authorization

Customer is on the highway; shop texts the link. Customer signs on their phone for the work to proceed.

Estimate-overage authorization

Repair grew beyond the initial estimate. Customer signs the same form again to authorize the additional cost.

Overnight or extended-stay vehicle

Storage-fee block surfaces. Customer acknowledges the per-day fee.

Questions we get a lot

Operator and signer questions about security, integrations, jurisdiction, and configuration.

Is the submission data secure?
Yes. Every submission is encrypted in transit and at rest. Add a custom subdomain on paid plans so the form lives on your own URL through the entire flow.
Can I capture a signature on the release?
Yes. The template includes an e-signature field, and the signed PDF is generated automatically on submission. Both parties can countersign if your workflow needs it.
Can I auto-send a copy to the signer?
Yes. Turn on Autoresponder Email to send the completed PDF to the signer the moment they submit. No manual follow-up required.
Can I route every submission to my system of record?
Yes. Push each submission via a webhook for real-time delivery, or use Zapier to fan out to your CRM, e-vault, and ops stack. Google Sheets sync works if you just need a log.
Does the release cover the estimate-to-final-cost change?
Yes. The form captures the initial estimate; the customer pre-authorizes work up to a defined dollar threshold above the estimate (e.g., +15%). Anything beyond that requires a second authorization signature.
Can the customer authorize remotely?
Yes. Text the customer the link from the bay; they sign on their phone and the work proceeds. The PDF carries the timestamp and signed authorization.
