Personal Training Liability Waiver & Consent

A personal training waiver covering health screening, training goals, scope of service, acknowledgement of risk, and signed release. Exported as a signed PDF for the trainer's file.

Releases & Waivers

About this template

This personal training waiver template runs on Formester's drag-and-drop form builder, sized for solo personal trainers and small coaching businesses. Lock in the client's health screening, goals, scope of service, and signed release before the first session.

Replace the paper packet. Clients fill the waiver online before their first session, sign with e-signature, and the PDF lands in the client file. Encrypted in transit and at rest.

Use conditional logic to surface scope-specific blocks (nutrition coaching, mobility, post-rehab), reveal the in-person versus virtual disclosure depending on session type, or include the doctor-clearance block when health screening flags a condition.

Auto-send the signed waiver back to the client via Autoresponder Email, route every submission to your coaching CRM via webhooks, and keep one searchable history per client.

How to customize this personal training waiver

Six steps to a coaching-ready waiver.

Brand the header

Replace the placeholder organization name, logo, address, and contact details with your brand. Set primary colors so the form matches the page it lives on.

Set scope of service

Add the services you offer (training, nutrition coaching, mobility, recovery). Each surfaces its own disclaimer block.

Update the health screening

Edit the conditions checklist (heart, BP, chronic pain, recent surgery, pregnancy). Add notes fields for context. Add the doctor-clearance block for flagged conditions.

Define session-type disclosures

Add in-person, virtual, and hybrid options. Virtual surfaces a separate disclaimer (no in-person supervision of form or equipment).

Wire delivery

Push every submission to your CRM or e-vault through a webhook, or use Zapier for fan-out to email, Slack, and notifications. Turn on Autoresponder Email if the signer should receive a copy.

Publish and share

Embed the form on your site, share the direct link by email, or generate a QR code for in-person signing. Every submission lands in the dashboard with a clean PDF for the file.

Questions we get a lot

Operator and signer questions about security, integrations, jurisdiction, and configuration.

Is the submission data secure?
Yes. Every submission is encrypted in transit and at rest. Add a custom subdomain on paid plans so the form lives on your own URL through the entire flow.
Can I capture a signature on the waiver?
Yes. The template includes an e-signature field, and the signed PDF is generated automatically on submission. Both parties can countersign if your workflow needs it.
Can I auto-send a copy to the signer?
Yes. Turn on Autoresponder Email to send the completed PDF to the signer the moment they submit. No manual follow-up required.
Can I route every submission to my system of record?
Yes. Push each submission via a webhook for real-time delivery, or use Zapier to fan out to your CRM, e-vault, and ops stack. Google Sheets sync works if you just need a log.
Can the waiver scope to nutrition coaching if I offer it?
Yes. Surface a scope-of-service multi-select (training, nutrition guidance, mobility, recovery). Conditional logic ensures nutrition-specific disclaimers (not a registered dietitian, education only) appear when nutrition is selected.
Does this work for in-person and virtual sessions?
Yes. The session-type dropdown surfaces a virtual disclaimer (no direct supervision of form, equipment safety responsibility) when virtual is selected.
