About this template

This reference check form runs on Formester's drag-and-drop form builder, so you can adapt the rating questions, role-specific competencies, and consent language to your hiring workflow without writing code. Captures structured feedback from professional references: relationship, role context, performance ratings on key competencies, and honest narrative response.

References fill the form on any device, rate the candidate on structured competencies, write narrative responses, and sign with e-signature. Each submission produces a clean PDF for the candidate file, and the structured ratings power a comparison view across all references the candidate provided.

Use conditional logic to surface management-specific questions when the reference managed the candidate, peer-specific questions for colleagues, and direct-report questions for candidates the reference reported to. Send each reference an acknowledgment with Autoresponder Email so they know the response was received.

How to use this reference check form

Seven steps to a working reference workflow. Most hiring teams finish in under 15 minutes.

Open the template

Click Use Template. Rating scales, conditional rules, and signature flow load into the Formester builder.

Brand the form

Drop in your company logo and candidate context so references know who is asking and about which role.

Configure competencies

Edit the rating questions to match the role: leadership for management roles, technical depth for engineers, customer empathy for support roles.

Apply conditional logic

Use conditional logic to surface manager-specific questions when the reference managed the candidate, peer questions for colleagues, and direct-report questions for the candidate's former managers.

Set the rating scale

1 to 5 or 1 to 10. The form averages ratings across competencies and across references so the hiring team has a quick at-a-glance comparison.

Capture signed authorization

E-signature on the reference statement and authorization to release information. Required for legal defensibility of the reference data.

Compile across references

Each candidate provides 2 to 4 references. The dashboard groups responses by candidate so you can compare ratings and narrative across every reference.

Questions we get a lot

HR, hiring manager, and reference questions about structure, anonymity, and use of the data.

Is the reference data legally usable in hiring?
The form captures the standard elements (relationship, role context, ratings, signed authorization). Have HR or legal review the language for your jurisdiction; reference checks have specific FCRA and ADA considerations.
Can I customize the rating questions per role?
Yes. Edit the competency list per role. Use conditional logic to surface manager-specific competencies when the reference managed the candidate, peer competencies for colleagues, and direct-report perspectives for former managers.
Can references stay anonymous?
Anonymity is not recommended for legal defensibility. The form requires reference identity for the file. If your process supports anonymous-aggregate-only review, surface that path through a separate form.
Can I compare ratings across multiple references for the same candidate?
Yes. Each candidate provides 2 to 4 references. The dashboard groups responses by candidate so you can compare structured ratings and narrative side by side.
Can I send the request to references via the candidate?
Yes. The candidate sends the form link to their references; references fill independently. Or the hiring team sends directly to references the candidate listed in their application.
Where are reference records stored?
Every submission saves to your dashboard with a PDF copy. Filter by candidate or reference, and export the archive for the hiring file.
