Use this when you have an in-person or virtual event and need name, email, session choice, and dietary preference. The Google Forms version covers the basics. Add the email autoresponder by enabling “Collect email addresses” in Settings, then “Receipts” in the Responses tab.

Cleanup checklist when you copy it: change the event title, swap session names, set a response cap if seats are limited (Forms doesn't ship this natively, so it's a manual cutoff). For small-team hiring (1-3 open roles, under 100 applicants per role), this template is fine. Name, contact info, role, resume upload, portfolio link, salary expectation, availability. Set resume upload as required and cap file size at 10 MB so Google Drive doesn't fill up with 100-page PDFs.

The honest limit: Google Forms can't score or rank applicants. You'll export to a Google Sheet and triage manually. 4. Customer feedback form

A 5-question feedback form gets a 3-4x higher completion rate than a 15-question one (SurveyMonkey research on survey length). This template ships five: overall satisfaction, what you liked most, what could improve, likelihood to recommend (NPS 0-10), and a free-text open question. Don't expand past 8 unless you're paying respondents.

The NPS question uses Google Forms' “linear scale” set to 0-10. Label the ends “Not at all likely” and “Extremely likely” exactly. This is two different jobs glued into one template. Surveys collect opinions (no right answer). Quizzes test knowledge (graded). Google Forms supports both via the “Make this a quiz” toggle in Settings, but the UX is shared, which is why both end up in the same template.

For a teacher running a weekly comprehension check, the quiz mode is fine: set the toggle, add point values per question, set correct answers, share. Auto-grading lands in the spreadsheet. For a market researcher running a customer segmentation survey, leave the quiz toggle off and treat it as a normal Google Form.

How to use a Google Forms template in 3 steps

The walkthrough below applies to every template in this guide and every template in Google Forms' native gallery.

  1. Open the template

    Click “Make a copy” on the Google Forms link. The form lands in your Google Drive as a new file. If you're not signed in to Google, you'll be prompted before the copy completes.

  2. Customize the title, questions, and settings

    Change the title and description at the top. Edit each question by clicking it; the side panel lets you change question type (short answer, multiple choice, dropdown, linear scale, file upload, etc.). Mark questions required by toggling the asterisk. For settings (collect email addresses, allow editing after submit, confirmation message, response receipts), click the gear icon in the top-right.

  3. Share
